staging: unisys: refactor DEVICE_MAP 



Get rid of the typedef for DEVICE_MAP and use struct device_map instead.
Fix CamelCase names.

DeviceChannelSize => device_channel_size
CA_Index => ca_index
Reserved => reserved
Reserved2 => reserved2
